Combined orthodontic and surgical treatment

Combined orthodontic and surgical treatment (orthognathic surgery) is not just a complicated term, but also a highly precise treatment process made up of several carefully coordinated steps. So that you know exactly what to expect, we have divided the entire process into 12 clear steps for you – from the first consultation to aftercare.

Step 1

Initial consultation

Initial consultation with your orthodontist. This includes a clinical examination and your medical history. We explain the next steps to you in detail, and you receive a provisional treatment and cost plan. The appointment is free of charge and takes approx. 20 minutes.

Step 2

Diagnostic records (records appointment)

Compiling your records: at this appointment, we take a 3D scan, facial photos and intraoral photos as well as a cephalometric X-ray and a panoramic X-ray. From the age of 18, this appointment is subject to a fee; it takes approx. 30 minutes.

Step 3

Analysis (consultation)

Based on the findings, the orthodontist plans your treatment, which we then discuss with you in the analysis consultation. You receive the final cost estimate. We also support our adult patients in submitting their claims to statutory (public) and private insurance. Duration approx. 20 minutes.

Step 4

Consultation with the oral and maxillofacial surgeon

During this consultation, the surgical plan is confirmed or adjusted, you are informed in detail about the possible risks of the procedure and – if you wish – a surgery date is reserved without obligation.

Step 5

Bonding the braces

Pre-operative orthodontics begins with the bonding: the fixed braces are fitted, and the appointment takes about an hour. The overall duration of the orthodontic treatment varies depending on the malocclusion, the type of brace and the surgical approach.

Step 6

Follow-up at the dysgnathia clinic

At around 4/5 of the way through treatment, you return to the dysgnathia clinic (the specialist clinic for jaw malpositions). The surgery date is fixed, the risks and the procedure are discussed, and clearance for surgery is arranged via the AKH (Vienna General Hospital) or your GP.

Step 7

Virtual surgical planning

Once pre-operative orthodontics has been completed, the digital surgical planning takes place using 3D scans, CBCT data and calibrated facial photos. The planning is carried out by the oral and maxillofacial surgeon – usually via TeamViewer or directly in the Dolphin software.

Step 8

Internal clearance for surgery

Arranging the blood tests and the consultation with an anaesthetist. Appointment for internal clearance at the AKH or with your GP.

Step 9

Countdown to surgery

1–2 days before the operation, surgical hooks are attached and the brackets are fitted with wire ligatures. The surgical splints are planned and produced beforehand using CAD/CAM in the digital Eltz lab.

Step 10

The operation

On the day of the operation, please be sure to bring the surgical splint with you into the operating theatre – it is essential for the surgical procedure. Depending on the extent of the operation and your individual situation, the hospital stay after the procedure varies between 1 and 7 days.

Step 11

Fine-tuning

The fine-tuning is carried out with orthodontic treatment. You can be treated with fixed braces, but also with aligners. The fine-tuning often takes only a few months.

Step 12

Completion of active treatment

Fitting of the appropriate retention appliances. After active orthodontic treatment, it is necessary to wear some form of retainer for life. This is a removable night-time brace. In addition, a fixed wire can also be bonded to the inside of the teeth.
